Dr. Caraballo-Burgos is board certified in Family Medicine and serves as Chief Medical Officer for Florida.  He brings more than 20 years of diverse leadership experience as a physician with significant achievements in developing care – taking strategies, coaching medical staff on healthcare business, managed care, home care visits and medical practice issues. Dr Caraballo’s served as a Senior Vice President and Medical Director for a very large Multi- State Healthcare Organization, dedicated to the provision of home integrated services to include home health skilled nursing care modalities, durable medical equipment including ventilators as well as infusion specialty pharmacy services for over 4.5 million members in 7 states.

Over the last 20 years, Darwin has held other leadership roles across the industry, including positions as the Chief Medical Officer for MMM of FL Medicare Advantage, Medical Director/SVP for Hygea MSO with Medical Offices in FL & GA, Medical Director for Aetna Better Health of Florida – Medicaid and Long Term Care Plans, Magellan Complete Care – SMI Medicaid Specialty Plan, Simply Healthcare Plans Medicare Advantage Plans, Better Health Plans of Florida - Medicaid, Clear Health Alliance – an HIV Medicine Medicaid Specialty Plan and Leon Medical Centers Health Plans Medicare Advantage.

Dr. Caraballo graduated from Halifax Medical Center Family Medicine / Sports Medicine Residency Program in Daytona Beach, FL and from Ponce School of Medicine in Puerto Rico.
